* – NOTE: Affiliate members can petition the NMTA Health Trust Board and EPK & Associates for participation in the NMTA Health Trust. To participate in the NMTA Health Trust, Affiliate members must meet not only By-Law membership criteria but also additional requirements established by the United States Department of Labor and the Washington State Office of the Insurance Commissioner.
